Plot
Robert McCall murders criminal Lorenzo Vitale and his goons at a remote Sicilian winery in order to get a key to the winery’s vault and recover money that was taken in a cyberattack. McCall is shot in the back by Vitale’s grandson as he leaves the winery. Because of the injury, McCall considers suicide, but he changes his mind and boards the ferry to return to the mainland.
McCall stops while driving on the Amalfi Coast and passes out from shock. He is encountered and saved by Gio Bonucci, a local carabiniere, a kind of law enforcement officer. He takes McCall to the isolated Italian seaside village of Altamonte, where he receives medical care from Dr. Enzo Arisio.
McCall gets to know the residents and develops a fondness for their village while he recovers from the shooting. He calls CIA agent Emma Collins anonymously to inform her of the winery’s covert involvement in the illicit narcotics trafficking. McCall’s suspicions are confirmed when Collins and other CIA agents find large sums of money and bags of fenethylline tablets concealed in a cupboard. They continue to work together once she finds McCall in Altamonte.
In the meantime, the Camorra, led by brothers Vincent and Marco Quaranta, intimidate and murder peasants in an effort to force them from their homes and seize Altamonte for commercial gain. McCall hears Marco coercing Angelo, the owner of a nearby store, into making payments—a tactic akin to extortion. As the town looks on, the Camorra firebombs Angelo’s fish store as an example.
Gio calls the Italian central police for an investigation after watching the firebombing footage. The Camorra attack and beat Gio for interfering with their operations after capturing his wife and daughter to entice him. A police source had warned them. In order to expand his operations, Marco insists that Gio arrange for him to have a boat with contacts in Somalia. After listening in on the talk, McCall requests that Marco relocate his business so that the residents of the town are left alone. McCall murders him and his goons when he refuses.
The chief of police in Naples is threatened and tortured by an angry Vincent, who also gives him orders to track down Marco’s killer. Vincent plants a bomb in Collins’ car, but she survives the explosion and ends up in the hospital. When Vincent comes to Altamonte, he threatens to kill Gio in front of the locals unless Marco’s murderer is found. When McCall shows himself, the populace starts to object, using their cellphones to record Vincent and his goons as he gets ready to put McCall to death. Vincent withdraws with his group. They reassemble at his estate, preparing to strike back the next day.
McCall breaks into Vincent’s mansion that evening. He gives Vincent a fatal dose of the drugs that Vincent has been peddling all throughout Europe after killing each of Vincent’s bodyguards one by one. McCall uses Collins to provide money he stole from the winery to an elderly couple whose pension fund was stolen and hacked.
Collins is promoted to CIA headquarters in recognition of her role in putting an end to the drug trade in Altamonte. She gets a gift from McCall with a note and a book of contacts. Collins is the daughter of McCall’s late friend and coworker Susan Plummer, according to the note, which congratulates her and says her mother would be proud of her.
McCall rejoices with the Altamonte residents following their football team’s triumph as the threat posed by the Quaranta brothers has been eliminated.
