Nicknamed “Shankar Dada” (roughly, “Brother Shankar”), Shankar Prasad is a Hyderabadi crime don, often known as a bhai or goonda. Since his father Satya Prasad had wanted him to become a doctor, he establishes the fictitious Sri Satya Prasad Charitable Hospital in his father’s honour and acts as though he lives up to this wish every time his parents come to visit him in Hyderabad.
Shankar’s plan, however, is derailed one day when his father runs into Dr. Ramalingeswara Rao, an old acquaintance, and the two older men agree to marry Shankar to Dr. Sunitha “Chitti,” the daughter of Ramalingeswara Rao. At this moment, Shankar’s true nature becomes apparent. For not knowing about Shankar’s real life, Ramalingeswara calls Shankar’s parents “fools” and makes fun of them. Shocked and then devastated, Shankar’s parents return to their village. Devastated and hopeless, Shankar determines that becoming a doctor is the only path to self-redemption and retaliation for his father’s humiliation at the hands of the vengeful Ramalingeswara. In order to earn an MBBS degree—the graduate medical degree in South Asia—he chooses to enrol in a medical college.
Shankar eventually has enough shame to drop out of college as his remorse over not being able to save a dying buddy overcomes him. Soon after Shankar leaves, Sriram emerges from his vegetative state by miracle. Sunitha then delivers a moving speech in which she berates her father for sending Shankar away, claiming that doing so is akin to expelling happiness, compassion, love, and hope from the college. Eventually, Ramalingeswara comprehends his foolishness. After Sunitha and Shankar get married, Shankar discovers for the first time that she is “Chitti”. When his parents do get back together, Shankar Prasad becomes Shankar Dada MBBS.
